From dBA to soundscape indices: Managing our sound environment

Abstract: While in the EU alone 80 million citizens are suffering from excessive environmental noise, the conventional approach, i.e., reduction of ‘sound level’, does not always deliver the required improvements in quality of life. The growing field of ‘soundscape studies’ is addressing this gap by considering the sound environment as perceived, in context, with an interdisciplinary approach. However, soundscapes are hugely complex, and measuring them as a basis for environmental design requires a step change to the discipline. This paper explores the need for developing ‘soundscape indices’, in the movement from noise control to soundscape creation, adequately reflecting levels of human comfort, the impact of which will be reminiscent of that of the Decibel scale created by Bell Systems a century ago. By analysing the soundscape design of urban open public spaces, the coherent steps for achieving this are also discussed, including characterising soundscapes by capturing soundscapes and establishing a comprehensive database; determining key factors and their influence on soundscape quality based on the database; developing, testing and validating soundscape indices; and demonstrating the applicability of the soundscape indices in the management of our sound environment.

Advances in managing hepatocellular carcinoma

Abstract:

Multiple modalities for treatment of hepatocellular carcinoma are available, depending on tumor size and number. Surgical resection remains the gold standard, so long as the residual liver function reserve is sufficient. In patients with advanced cirrhosis, liver transplantation is the preferred option, as these patients may not have adequate hepatic reserve after resection. Salvage liver transplantation has also become an option for a select few patients who recur after surgical resection. Ablative techniques have been used for palliation as well as to either completely destroy the tumor, act as an adjunct to resection, or downstage the tumor to meet Milan criteria such that a patient may be a candidate for liver transplantation. Radiofrequency ablation, microwave ablation, chemoembolization, radioembolization, and irreversible electroporation have all been used in this capacity. Currently, sorafenib is the only US Food and Drug Administration-approved chemotherapeutic for hepatocellular carcinoma. The efficacy of sorafenib, in combination with other agents, transarterial chemoembolization, and surgical resection is currently being investigated. Sunitinib and brivanib, tyrosine kinase inhibitors, have failed as potential first- or second-line options for chemotherapy. Bevacizumab in combination with erlotinib is also currently being studied. Final analysis for ramucirumab and axitinib are pending. Tivantinib, a selective mesenchymal-epithelial transition factor (MET) inhibitor, is also undergoing clinical trials for efficacy in MET-high tumors. This review serves to emphasize the current and new technologies emerging in the treatment of hepatocellular carcinoma.

Current recommendations of managing HBV infection in preconception or pregnancy

Abstract:

Hepatitis B remains a leading cause of cirrhosis, hepatocellular carcinoma and liver transplantation worldwide. Management of chronic hepatitis B during pregnancy is challenging. Transmission of hepatitis B to infants still occurs perinatally although immunoprophylaxis is widely available for infants born to mothers with chronic hepatitis B infection. The emerging data suggest that initiation of antiviral therapy in the beginning of the third trimester in highly viremic mothers can prevent immunoprophylaxis failure in their infants. The available drug safety data show that lamivudine, telbivudine and tenofovir are generally safe to be used during the pregnancy. In order to minimize the fetal exposure to the antiviral medication, antiviral therapy during the pregnancy should be limited to a selected group of patients with cirrhosis, high hepatitis B viral load, or prior history immunoprophylaxis failure. An elective Caesarean section may reduce the risk of perinatal transmission. For those females planning for pregnancy or in early stage of pregnancy, communication and follow-up among obstetrician, gastroenterologist, and primary care physician are important. In this article, we will review the features of hepatitis B infection before, during and after the pregnancy; the risk factors that increase mother-to-child transmission; safety data on antiviral drug use during pregnancy; and the potential role of Caesarean section in selected cases.

This paper traces the development of the concepts most project managers take for granted including bar charts and critical path schedules from their origins (which are far earlier than most people think) through to the modern day. Abstract: Stormwater runoff at airports is a significant and costly issue, especially for the stormwater laden with deicing contaminants of high Biochemical Oxygen Demand (BOD) and aquatic toxicity. To reduce the loading of deicing constituents in stormwater and to manage the increasing pressure of tightening regulations, identifying fate and transport and evaluating environmental risks of deicing stormwater are of critical importance. In this review, the regulatory development of airport deicing stormwater management was first discussed, along with the milestone publication. The deicer usage and fugitive losses can be reduced and the amount of deicer collected can be increased by having a better understanding of the fate and transport of deicing constituents in stormwater. As such, an overview and evaluation of the constituents of concern in deicers were provided to support the assessment of environmental impacts and mitigation recommendations. The state of knowledge of airport deicing stormwater management was then reviewed, which needs to be synthesized into a national guidance document. A guidebook and a decision tool for airports were proposed to adopt specific practical stormwater management strategies while balancing their priorities in environmental, economic, and social values against operational constraints. These challenges pose great opportunities to improve the current practices of airport deicing stormwater management.

Managing Coordination Complexity in the Remanufacturing of Aircraft Engines

Abstract: The complexity of communication and coordination stemming from teams responsible for adjusting interdependent parameters of components is a fundamental feature in the aircraft engine remanufacturing engineering project. To manage coordination complexity, the features of the remanufacturing process of aircraft engine are analyzed and a systematic method is presented to measure and optimize the dependency between coupled components. Furthermore, quantitative models are built based on Design Structure Matrix (DSM) models to measure dependency strengths related to the parameter features of the components. Also, a two-stage DSM clustering criteria is used to reduce the complexity of an organization. An industrial example is provided to illustrate the proposed models. The results showed that the proposed approach can reduce total coordination complexity.
